Who should report a death?
An emergency contact or family representative should notify DCMA.

Information to have ready
Please provide the following information, if available:
- Employee’s full name
- Organization or office (if known)
- Date of death
- Your name and relationship to the employee
- Your phone number and email address

A DCMA representative will contact you to discuss administrative matters.
